The Coleman Foundation Announces Fourth Quarter 2018 Cancer Care and Health Rehabilitation Services Grants

In the fourth quarter of 2018, the Coleman Foundation approved grants totaling $2,683,050 to nonprofit organizations in support of health and rehabilitation including cancer care.

Receiving grants in this program area are:

Cancer Care

 

Northwestern University, IL     

$1,002,300 – Coleman 4R Patient Care Sequences for Cancer and Supportive Care

This award is to support grant management and administration for participating institutions of the Coleman Supportive Oncology Collaborative to develop and implement the Coleman 4R Patient Care Sequences for Cancer and Supportive Care. The 4R model is designed to work with oncology care teams to implement a process which results in providing cancer patients with care planning documents at diagnosis and adjusted throughout the continuum of care. This award includes support for Working Groups and Advisory Teams; content development; and program related expenses for core program support. The award also supports Northwestern as a participating site along with seven other institutions.

Health Rehabilitation

 

Center for Independence through Conductive Education, IL

$175,750 – Conductive Education Center in Chicago

This award is to support building renovations for the new Conductive Education Center in the Chicago, Brighton Park community. The new site, slated to open in 2019, will deliver Conductive Education and Vocational Training services to children and young adults with physical disabilities, particularly cerebral palsy. Children and families previously served at the El Valor Conductive Education program in Chicago’s Pilsen neighborhood will continue to receive services at the new location in Brighton Park.

 

Center for Independence through Conductive Education, IL  

$50,000 – General Operating Support

This award support programming to help children and adults with physical disabilities by offering Conductive Education, which breaks down physical skills like walking, sitting and eating into achievable tasks and requires every participant to take some responsibility for their learning. Participants initiate tasks, take a risk, cope, persevere, adapt – and learn with an “I Can!” attitude.

 

Devices 4 the Disabled, IL         

$130,000 – Ed Kane Clinic for Disabled Support Services

This award is to support Devices 4 the Disabled in providing durable medical equipment at no cost to disabled persons who are uninsured, underinsured, or undocumented. Specifically, this award supports the Chicago facility renovations; creation of the seating and positioning clinical area, in which an Assistive Technology Professional will conduct assessments to customize wheel chairs and power chairs; and staffing.

 

Latino Alzheimer’s & Memory Disorders Alliance (LAMDA), IL       

$50,000 – Cicero/Berwyn Site Support

The Latino Alzheimer’s and Memory Disorders Alliance (LAMDA) was founded in 2008.  Its mission is to educate and engage Latino caregivers of family members facing the challenges of Alzheimer’s and Memory disorders of by providing skill building training, and compassionate support programs. These grant funds support continued direct service programming at the Cicero/ Berwyn site.
