The Coleman Foundation Announces Second Quarter 2019 Developmental Disability Grants

In the second quarter of 2019, the Coleman Foundation approved a grant totaling $775,000 to a non-profit organization advancing strategies articulated in the Foundation’s Disabilities Impact Plan.

$775,000 – Community Hub Initiative & VBR Initiative

The award is for the second phase of Envision’s Community Hub Initiative which is bringing its community day services in line with best practices for maximizing client choice and community integration. The Initiative disaggregates Envision’s large community day centers which had as many as 150 clients receiving services under one roof, transforming them into smaller community hub sites that serve about 50 clients or fewer. During Phase Two of the Community Hub Initiative, Envision will open new hubs near two existing centers to reduce the census at these sites while still having locations in the areas where clients live.

Also funded through this award, Envision will launch a Value Based Reimbursement pilot initiative in partnership with UCP Seguin of Greater Chicago though which the two organizations would study the impact of its programmatic services on hospital days and readmission rates for a specific pool of approximately 50 clients served by the two agencies.  The organization will track client medical outcomes for these individuals, in partnership with a major health insurance company.
